Social Services Coordinator – Refugee Care

Job Summary:
Family Health Centers of San Diego is looking for a Social Services Coordinator for Refugee Care. Under the direction of Social Services Leadership, the Social Services Coordinator will support Refugee patients entering our Primary Care System by addressing social determinants of health, providing referrals for support services, and guiding patients through FHCSD health system through episodic and longitudinal care management.

Responsibilities:
Provide critical navigation services to patients and their family members, with exceptional customer service.

Assist and help in identifying needs of the patient for a successful primary care visit through facilitating complete discharge planning.

Perform patient assessments under to determine the appropriate level of care management.

Serve as a critical member of the multidisciplinary Care Team.

Supports Clinic flow by accurately collecting Social Determinants of Health, Sexual Orientation and Gender Identity, in addition to performing other essential screenings to uncover patient needs and vulnerabilities.

Receives referrals and warm handoffs from the extended clinic care team.

Participates in rounding and case conferencing to develop linkages to patients in need.

Create care goals with the patient, and/or patient/caregiver as appropriate, includes development of SMART goals to monitor interventions and progress.

Provide basic individual support based on patient’s needs through episodic care management and refers to support programs as appropriate. .

Maintains a longitudinal care management caseload of roughly 15-20 patients.

Effectively manage patient experience time to include exercising exceptional time management and communication skills.

Provide documentation of all encounters, assessments, and provided support.

Maintain good working relationships with community partners and utilize the community resources to connect new and existing patients to FHCSD.

Educate patients and their families on resources and problem-solving strategies on topics such as abuse, neglect, patient rights and responsibilities, conflict resolution, and community health and social services available.

Converse and completely adhere to HIPAA regulations.

Perform other duties as assigned.

Requirements:
Bachelor’s Degree in Social Work Required. Master’s Degree in Social Work strongly preferred.

(3) years of experience in a healthcare setting.

Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.

Bilingual in English/Arabic highly desired (language skills/competency will be tested).
