Working hours: 8 hours per day from Sunday to Thursday

Responsibilities and Tasks: 

Main Case Manager responsibilities:

·       Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered.

·       Ability to both see both needs related to individual issues and the wider context, this containing both social, familiar, community and cultural matters.

·       Handle case assignments, draft service plans, review case progress and determine case closure

Referrals

·       Coordinate with other relevant NGO or governmental organizations and services available for each case.

·       Have knowledge of the official referral pathway and refer cases when needed

·       Work according to the goals, values and rules of conduct of Yazda.

·       Facilitate multiple care aspects (case sharing, etc) coordination, information sharing.

Case Manager – counseling

·       Help clients achieve wellness and autonomy

·       Assess and address motivational and psychosocial issues, including basic stabilization techniques 

·       Reduce stigma and shame by facilitating general psychoeducation /awareness

·       Create social support through community amongst the clients

·       Facilitating psychosocial support through individual/ groups sessions and recreational activities

·       Record cases information, complete all necessary forms accurately.

·       Adhere to professional standards as outlined by protocols, rules and regulations

·       Ensure confidentiality and privacy for all beneficiaries

Preferred skills and knowledge:

·       Degree in psychology is highly desirable

·       At least 2 years of previous experience in Case Management
