Dear Sir/Madam,  

On behalf of the Syrian American Medical Society Foundation, I am kindly requesting quotation  for the attached list  

Below are applicable RFQ terms: 

Quotation Validity: 

Quotation should be valid for 30 days from the RFQ due date. This includes, but is not limited  to, cost, pricing, terms and conditions, service levels, and all other information. If your firm is  awarded the contract, all information in the RFQ and negotiation process is contractually  binding. 

Delivery Location and Terms:  

Delivery of the RFQ must be send to no later than 30th of November  2021.  

Partial quotation: 

SAMS will consider partial offers and partial bids.  

Quality requirements: 

the bidder must provide his/her resume or the company resume or any other related document  proving of being a legal firm the attached list below.  

Payment terms: 

Value of items will be paid 30 days after delivering the required items from the bidder, and  acceptance of all tasks by SAMS. 

Response deadline: 

The deadline to respond is 30.11.2021 Quotations received after stated deadline may be  considered invalid.

Terms and Conditions 

– Issuance of this RFQ does not constitute an award commitment on the part of the SAMS, nor  does it commit SAMS to pay for costs incurred in the preparation and submission of a bid.  

– SAMS may contact bidders to confirm contact person, address, bid amount and to confirm that  the bid was submitted for this solicitation. 

– False Statements in the Bid: 

Bidders must provide full, accurate and complete information as required by this solicitation and  its attachments. 

– Reserved rights: 

All RFQ responses become the property of SAMS and SAMS reserves the right in its sole  discretion to:  

• To disqualify any offer based on Bidder’s failure to follow solicitation instructions; • To waive any deviations by Bidder from the requirements of this solicitation that in SAMS  opinion are considered not to be material defects requiring rejection or disqualification; or  where such a waiver will promote increased competition; 
• Extend the time for submission of all RFQ responses after notification to all Bidders; • Terminate or modify the RFQ process at any time and re-issue the RFQ  • Award only part of the activities in the solicitation or issue multiple awards based on  solicitation activities. 

Source Selection Criteria 

Based on the lowest price technically acceptable source selection process, award will be made  to the bidder submitting the lowest evaluated price that meets or exceeds the technical  acceptability standards while respecting determined delivery timelines. Technical capability  may be evaluated by how well the proposed products meet the minimum technical  specifications set forth in this RFQ.

• calculation of employee’s social  security and income taxes   according to GOI regulation.  • Issuing the annual financial report in accordance with   applicable laws for GoI and KRG requirements by (Date). In this  report the Legal accountant will  verify and report on the accuracy  of the income tax and social  security amounts withheld by the  organization during the period  from (Date) to (Date) and will

certify that all payments owed to  the GoI and KRG were correct and  made in accordance with all  applicable Iraqi laws. The Legal  accountant will also confirm that  the organization did not make a  profit with its activities in Iraq.  • The Legal accountant will certify  the financial accounts for the  period audited in accordance with  the Iraqi rules and regulations and  will present them to the Iraqi  Union of Accountants and to all  relevant government authorities  requiring those reports as   required by Iraqi law of all legally registered organizations in Iraq.  • The Legal accountant will   regularly update and advise the  organization regarding current  Government of Iraq (GOI) rules  and regulations related to   operating as a registered entity in  Iraq, including any emerging  changes in the regulatory   framework.  • The Legal accountant will render  advice on financial issues,   including taxation and social  security and keep the organization  updated with any amendments or  modification in the respective  Laws.  • The Legal accountant will assist  in the submission of any   additional annual reports required  from registered entities under  Iraqi law.  • The Legal accountant will   provide other advice and   information as requested by the  organization.  • Review and legalize employees contract according to GOI and

KRG regulations
