DESCRIPTION OF DUTIES / ESSENTIAL FUNCTIONS

Houston Health seeks an individual skilled in working as a liaison with Houston Health Department other city Departments, public interest groups and diverse ethnic groups in the community . The responsibilities for this position are as follow:

Core job duties for Community Liaison:
Provides oral and/or written reference services and information assistance to the community at large.

Develops and maintains a filing and tracking system covering disseminated information, referrals, requests, and complaints.

Performs site visitations and surveys items/areas of complaint.

Performs special assignments as requested.

Core job duties for Community Liaison for the Immunization program

Plans, organizes and participates in health events and activities to promote immunization (health fairs, presentations, trainings, national observances, etc.) during standard business hours and after hours.

Conducts immunization education monthly to increase awareness of vaccine-preventable diseases throughout the city of Houston and Harris County.

Works with doctor’s offices and health clinics to provide education and interventions to increase the use of best practices, knowledge of vaccine storage and handling and policies related to the Texas Vaccines for Children Program.

Develop, implement and evaluate new initiatives to improve immunization coverage rates of children, adolescents and adults.

Completes data entry of activities conducted and reports in a timely manner and as assigned.

Conducts monthly audits and quality assurance checks to ensure proper implementation of Project Milestone activities among Health Centers and WIC sites.

Other duties as assigned.

Additional Job Duties for Community Liaison in Office of Chronic Disease, Health Education and Wellness

Interfaces with neighborhood groups, community-based organizations, worksites, schools, and general public.

Maintains liaison with other community social agencies and industry-related programs; serves as a community resource person as required.

Provides oral and/or written reference services and information assistance to the community at large.

Facilitates key informant interviews, focus groups, listening sessions and other methods of grassroots qualitative assessments.

Facilitates workshops and trainings for volunteers, community leaders, and other partners on health equity, health literacy and accessibility to resources

Develops and maintains a filing and tracking system covering disseminated information, referrals, requests, and complaints.

Performs site visitations and surveys items/areas of complaint. Investigates and makes recommendations for addressing issues.

Represents the Health Equity initiative in community meetings, calls and stakeholder engagements.

Performs special assignments as requested.

WORKING CONDITIONS

The position is physically comfortable; the individual has discretion about walking, standing, etc.

MINIMUM REQUIREMENTS

EDUCATIONAL REQUIREMENTS

Requires a Bachelor’s degree in Liberal Arts, Business Administration or a related field.

EXPERIENCE REQUIREMENTS

Two (2) years of experience in community organizational work, resource and referral services, or a closely related field are required.

LICENSE REQUIREMENTS

A valid Texas Driver’s License and compliance with the City of Houston’s policy on driving (AP 2-2).

PREFERENCES

Below are preferences for those interested in the Community Liaison program in Office of Chronic Disease, Health Education and Wellness

Community Health Worker (CHW) certification is preferred

Bilingual in English, Spanish, Vietnamese, Chinese(Mandarin), Urdu, Arabic, French are preferred based on community needs.

Directly related professional experience in Community Outreach, Health Education, Social Work preferred

Experience delivering trainings and mediating workshops preferred

Able to work untraditional hours (after 5pm) and flex-time if needed

**Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6.**

SPECIAL FUND POSITION

Grants & Special funded positions are dependent upon continued available funds. If funding is no longer available, employee may be laid off or transferred.

Factors used in determining the salary offered include the candidate’s qualifications as well as the pay rates of other employees in this classification.

Pay Grade 18
