Health Education Resources Coordinator II

Job Summary

The Health Education Resources Coordinator II is responsible for the implementation of member health education service close loop referral systems, execution of member education campaign (through automatic calls and monthly member health education materials mailings) that support organization-wide health initiatives, and tracking and documentation of provision of health education services.

Duties

Implements health education service referral system: Triages all health education referrals to appropriate staff and referred members to community based social service agencies. Identifies and secured HIM class sites. Conducts member outreach to promote; confirmed class participation; and coordinated transportation as needed. Develops HIM and other member education calendar. Distributes member chart notes and documentation to providers. (60%)

Execution of member education campaign: Coordinates prenatal and smoking cessation mailing. Coordinates member outreach calls for flu, breast and cervical cancer screening, and other health priorities. Completes quarterly HE materials utilization reports. Inventories and orders HE materials. (10%)

Maintenance the CRD and HE materials: Surveys CBOs on annual bases. Coordinates revision of CBO information on annual bases and as needed. (10%)

Assists health educators and at health education classes, as needed: Prepares classes or assists health educators at health education classes. (10%)

Performs other duties as assigned. (10%)

Education Required

High School Diploma/or High School Equivalency Certificate

Education Preferred

Associate’s Degree

Experience

Required:
At least 0-2 years of customer service and/or healthcare experience.

Skills

Required:
Strong verbal and written communication skills.

Well organized with excellent skills in time management & attention to detail.

Considerable use of tact, diplomacy, discretion and judgment.

Proficient in Microsoft Office.

Preferred:
Bilingual in one of L.A. Care Health Plan’s threshold languages is highly desirable. English, Spanish, Chinese, Armenian, Arabic, Farsi, Khmer, Korean, Russian, Tagalog, Vietnamese.
