Position: Lead Case Manager

Location: Baghdad, Iraq

Reports To: TCRC Coordinator

  1. Project implementation support.
  • Provide case management services to clients, including conducting trauma-sensitive, survivor-centered assessments, identifying needs, and coordinating referrals based on the client’s needs;
  • Admits new clients by reviewing records and applications; conducting orientations.
  • Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
  • Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
  • Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
  • Develop action plans to address the needs of the client and follow up the provision of services;
  • Coordinate with members of the outreach/mobile team to provide key information necessary to ensure comprehensive services to the client;
  • Refer special cases to the case manager and coordinate appropriate response;
  • Work closely with the legal officers to coordinate with MoLSA officials on supporting detained children in juvenile facilities;
  • Work with the legal team and Consortium Partner specialized social workers in facilitating the process of connecting children with their families and provide support in obtaining legal documentation;
  • Develop case management policies and protocols and ensure training and capacity building for partners and staff members;
  • Maintain case documentation with care and confidentiality;
  • Support in training, coordinating, and monitoring caseworkers providing mobile outreach services;
  • Assist in the creation of tools, documents, and training materials for use in case management provided by partner staff;
  • Encourage and contribute to a safe and collaborative working environment where self-care as well as care of colleagues is prioritized.

 

  1. Communication and Monitoring & Evaluation
  • Collaborate closely with legal program officers, case manager, MHPSS coordinator and partner case managers to continuously assess status of the case against the case plan;
  • Ensure accurate data reporting and collection;
  • Supports compilation and writing of daily, monthly and quarterly updates.

  • Education and/or Experience:  Bachelor Degree or the equivalent and three years of related experience, or equivalent combination of education and experience.

·         Other Skills and Qualifications:  High degree of technical competency, attention to detail/accuracy and excellent organization skills required.  Ability to work in a fast-paced, high-productivity environment essential.

·         Language Skills:  Excellent oral and written Arabic and English

  • Computer Skills:  Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance.
