DESCRIPTION OF DUTIES / ESSENTIAL FUNCTIONS

The Houston Health Department seeks proactive candidates that will function as Community Health Workers/Navigators in the community engagement and outreach projects of the local COVID-19 vaccine response. Preference for applicants that are bi-lingual in English, Spanish, Vietnamese, Chinese (Mandarin), Urdu, Arabic, and/ or French based on the community needs. PHC/Community Health Workers will perform the following duties.

The Essential Functions are as follow:
Conduct community outreach and visits to various settings including churches, nursing homes, small businesses, worksites, civic groups, and others to promote vaccine accessibility, navigation, and COVID prevention.

Promote and schedules COVID-19 vaccine appointments for at-need community members and provides routine COVID prevention information.

Provide culturally and linguistically appropriate health education materials, and communicate in various languages as needed. Content includes but is not limited to COVID-19 vaccines, COVID-19 prevention, flu prevention, immunizations, and providing various pamphlets/information on management of underlying chronic disease conditions.

Maintain close working relationships with internal staff, community-based and private sector organizations to perform outreach and vaccine navigation activities.

Maintains daily counts and reports of number of encounters and outreach activities.

May provide assistance to non-English speaking community members.

May drive to perform duties.

WORKING CONDITIONS

The position routinely requires stooping or bending, frequent driving locally. Occasional very light lifting, such as three or four reams of papers or books (up to 20 pounds), may be required.

MINIMUM REQUIREMENTS

MINIMUM EDUCATION REQUIREMENTS

Ability to read, write, add, subtract, and follow oral and/or basic written instructions as might normally be acquired through 9 to 11 years of formal schooling. No special knowledge of any subject area or technical field is required.

MINIMUM EXPERIENCE REQUIREMENTS

One year of clerical experience is required.

MINIMUM LICENSE REQUIREMENTS

A valid Texas Driver’s License and compliance with the City of Houston’s policy on driving (AP 2-2).

PREFERENCES

Community Health Worker certification is preferred. Preference will also be given to individuals able to communicate in the following languages: Spanish, Chinese, Vietnamese, Arabic, French.

Able to work untraditional hours (after 5pm) and flex-time if needed.

**Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6.**

Grants & Special fund positions are dependent upon continued available funds. If funding is no longer available, employee may be laid off or transferred. Factors used in determining the salary offered include the candidate’s qualifications as well as the pay rates of other employees in this classification.
