FOR SUPPLY AND INSTALLATION OF MEDICAL EQUIPMENT FOR LOWER ARBAT PHC IN SULAYMANIYAH, IRAQ
QUOTATION TO BE RECEIVED BY: 03/11/2021 by 17:00 hrs (Iraq time)
The Office of the United Nations High Commissioner for Refugees (UNHCR) established on December 14, 1950 by the United Nations General Assembly 1, requests your price for Supply and Installation of Medical Equipment for Lower Arbat PHC in Sulaymaniyah, Iraq. As specified below in this Request for Quotation (RFQ)
The following Annexes form integral part of this RFQ:
– Annex A : SoW Scope of Work (please sign and stamp it to confirm its acceptance)
– Annex B : Technical Specification Form (please fill it, sign and stamp it to confirm its acceptance) – Annex C : Financial Offer Form/BoQ (bidders are kindly required to submit both signed & stamped PDF version along with filled excel sheet)
– Annex D : Vendor Registration Form (For suppliers who are not registered with UNHCR) – Annex E : UNHCR’s General Conditions of Contracts for the Provision of Mixed Goods & Service. – Annex F : UN Supplier Code of Conduct.
Please note that submitting an offer deemed as full acceptance of UNHCR’s General Conditions of Contract
- TECHNICAL REQUIREMENTS: (please submit your offer with the following):
– Registration Certificate
– Copy of bank account under business name
– Submit a copy of Product brochure and photo
– Warranty and maintenance as per Annex-B Technical Specification Form
Important notes:
– Supply and installation of equipment shall be completed with one month from the date of receiving Purchase Oder (PO) from UNHCR
– UNHCR may visit the supplier’s premises of all technically qualified suppliers before making its decision
– The medical equipment should meet the requested technical specifications, and approved by Directorate of Health engineering department and UNHCR public health unit
- TECHNICAL EVALUATION CRITERIA (Pass/Fail):
The technical offer will be evaluated based on a Pass/Fail criterion cascaded below:

- FINANCIAL OFFER:
Please submit your price offer, using financial form (Annex C).
UNHCR is VAT exempted, with this regard, price has to be given without VAT.
The fairly priced, substantially compliant offer will subsequently be recommended for a contract award.
Your quotation must be valid for at least 60 days. The standard payment terms of UNHCR is EFT net 30 days upon satisfactory delivery of goods/services and acceptance thereof by UNHCR.
UNHCR reserves the right to purchase the items by line items
