This job leads, develops, implements, and oversees services involved in managing patients and providers through value-based care delivery and clinical programs in the outpatient, post-acute and home setting. Strategically develops and implements programs for the company and our partners, including providing outreach, coordinating, and delivering care for patients, to manage the quality and cost of health care. Supports improving population health by developing and implementing strategic plans involving community partnerships and builds the network for post-acute facilities and services outside the company and in other markets where patients may access them.

Education
Required – Master’s degree in healthcare, business administration or related field.
Work Experience
Required – 10 years of experience leading or managing health service functions.
Preferred – Prior leadership experience in post-acute care modality, Medicare Advantage, CMS/Medicare/DOI regulatory experience. Prior experience with care management.

Job Duties
Provides strategic and operational leadership to assigned areas.
Develops new and innovative partnerships and programs that influence behavior of patients with chronic or complex conditions and provides support to them outside of the health system and in the home, with a focus on improving the quality of their health and total cost of care.
Oversees external affiliations and community partnerships as part of the company’s post-acute preferred provider network, locally and state-wide including, but not be limited to, long term acute care, skilled nursing facilities, nursing homes, inpatient rehabilitation, home medical equipment and durable medical equipment.
Effectively collaborates with post-acute care providers, partners, physicians, system leaders and care management to ensure that care is coordinated across the continuum.
Evaluates and advises senior leadership on program design improvement opportunities, new program opportunities, partnerships and collaboration opportunities in the outpatient, home care, and post-acute care environments.
Creates processes that address long-stay patients, post-acute resource referral lists, denials and determination of benefits.
Evaluates and advises senior leadership on program improvement opportunities, partnerships, and collaboration opportunities.
Develops, prepares, and manages annual approved budget, and recruits, hires and retains high performance employees focused on accomplishment of the mission.
Develops and implements programs that provide outreach coordinate care for patients in the home setting who need healthcare assistance and who can benefit from the program.
Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Performs other related duties as assigned.
