Refugee Health Caseworker

Description:
Seeking a candidate with experience in facilitating health and wellness initiatives to link newly resettled refugees and immigrants to healthcare resources with our Refugee Health and Wellness Program.

Full-time, 40 hours/week, competitive hourly wage, medical/dental/vision insurance eligibility and paid time off and paid holidays, flexible spending account (FSA) program, 403b tax-deferred retirement savings option, direct deposit, training opportunities, availability during office hours of Monday-Friday 8:30-4:30 with occasional evening and weekend availability for outreach/training activities

While CRIS staff are currently working remotely to prevent the spread of COVID-19, this position will require in-person contact in the workplace and with clients and others in the field. Applicants will be required to present proof of vaccination for COVID-19 after a conditional offer of employment has been extended. CRIS reasonably accommodates disabilities and sincerely held religious objections, and will engage in interactive dialogue to identify reasonable accommodations with those who have not been vaccinated for these reasons.

Utilizing a strengths-based perspective, work with the program team and other agency staff to coordinate health and wellness services that assist individuals with achieving self-sufficiency and accomplishing self-identified goals

Facilitate health and wellness assessment with individuals holding refugee or asylum status

Identify needs and facilitate receipt of available medical and community services for individuals in need of intensive case management

Work with program team to coordinate individuals’ receipt of immediate and long term medical and mental health support and services, including the coordination of transportation

Develop and maintain effective working relationships with local medical and mental health providers, referring individuals for specialized care when appropriate

Provide client transportation as needed (once in-person services resume)

Maintain regular contact with clients, including regular home visits (once in-person services resume)

Ensure provided services respect the rights, privacy and dignity of the individual and adheres to all contractual regulations

Participate in weekly Refugee Resettlement Program and weekly Refugee Health and Wellness Program staff meetings, serving as an engaged team member

Maintain data and detailed documentation on services provided

Collaborate with bachelor or master level student interns in the provision of program services

Perform other related duties to help meet the goals of the program and agency

. Requirements:
Minimum of a High School Diploma or GED

Skilled in interpersonal and intercultural communication

Ability to be self-directed with position responsibilities

Strong organizational skills

Advanced skill with Microsoft Excel, Google Apps including Google Drive, e-mail, and internet

Possess professional/academic abilities in English across all skill areas including clear pronunciation in speaking and correct grammar and spelling in writing

Experience facilitating partnerships with community medical and social service providers

Experience working with diverse populations

Experience working with limited English-speaking communities

Participate in required background check

Valid driver’s license, reliable vehicle, and current automobile insurance with clean driving record

Work Experience and Education Preferences:
Prefer bachelor’s or other four-year undergraduate degree in a social service, social work or health-related field

Experience providing case management to individuals and families

Experience working with interpreters

Preference for fluency in a language/s spoken by program clients (e.g. Arabic, Swahili, Kinyarwanda, Tigrinya)
